Passive Smoking and Cardiovascular Risk

Abstract

A possible relationship between passive smoking and coronaryheart disease has been widely debated during the past decade.Convincing evidence links environmental (passive) tobacco smokeexposure to heart disease morbidity as well as mortality. Inthe United States, 37 000 coronary heart disease deaths peryear are attributed to environmental tobacco smoke exposure,accounting for 70% of all deaths caused by environmental tobaccosmoke. The analysis of 10 epidemiologic studies indicated aconsistent dose-response effect related to exposure, but moreproof is still needed. Evidence indicates that nonsmokers aremore sensitive to smoke, including cardiovascular effects, andthat sidestream smoke contains higher concentrations of gasconstituents, including carbon monoxide. Pathophysiologicaland biochemical data after short- and long-term environmentaltobacco smoke exposure show changes in endothelial and plateletfunction as well as exercise capacity similar to those in activesmoking. Therefore, passive smoking is a relevant risk factorfor heart disease morbidity and mortality.
