The review article on doxycycline by Joshi and Miller1 was not only interesting but also valuableparticularly in this time of managed care and cost containment. Doxycycline is an effective and well-studied drug that is now quite cost competitivealthough I still remember when it was first released more than 20 years ago at the extraordinary cost of $1 per capsule.
In reviewing doxycycline, one should also address its pharmacokinetics. Doxycycline has a half-life of 15 to 22 hours.2 This means that it should be easy to maintain effective levels of the drug for a day or more after its administration. Since it absorbs well, 1 daily dose of doxycycline seems reasonable. If it is administered in the usual dosage of 100 mg twice daily, the serum concentration does not reach a steady state for days.
