The Trouble With Triglycerides

DESPITE DECADES of debate, the question of whether the triglyceride level is an "independent" risk factor for coronary heart disease (CHD) still generates considerable controversy. If, after all this time, we do not have an answer to this question, it is probably time to ask a different question. That question, addressed in part in the article by Avins and Neuhaus1 in this issue of the ARCHIVES, is not whether the triglyceride level is an independent risk factor for CHD, but whether it is a useful test for the clinician interested in the prevention and treatment of CHD.
A useful test is one that, at acceptable cost, convenience, and safety, affects diagnosis, prognosis, or therapy. For triglyceride levels and CHD, the issue is whether knowing a patient's triglyceride level has any therapeutic or prognostic implications.
