In their review of antipyretic therapy Plaisance and Mackowiak¹ fail to mention the role of such therapy in acute ischemic stroke. It is well recognized that the occurrence of fever is a poor prognostic indicator following stroke,^2-4 and is related to the size of the infarcted area.⁵ There is evidence that treating fever by inducing hypothermia reduces the degree of damage in animal models⁶ and improves outcomes markedly in humans.⁷ Experts in stroke treatment now recommend treating raised temperature with antipyretics.^8-9 We believe that the authors should have mentioned the role of antipyretic therapy in stroke.
