Do Anticoagulation Clinics Treat Patients More Effectively Than Physicians?

Most studies have shown that warfarin therapy administered by a pharmacist-run anticoagulation clinic is comparable to and more cost-effective than warfarin therapy administered in the usual fashion by a single attending physician.1-4 However, none of these studies compared treatment of the same patients. Furthermore, one of the most comprehensive studies1 can be criticized for comparing dissimilar patients (ie, a history of stroke was more common in the group of patients who were treated in the usual medical fashion). Our study is unique because it compares both types of warfarin therapy in the same patients.

Methods

A computerized medical database and the medical records of all patients who were treated with warfarin in the last 5 years at our facility were reviewed to select those who had their therapy initially administered in the usual fashion (by a single physician) for approximately 16 to 18 months and then by an anticoagulation clinic for . . .
