Sarcoidosis-Associated Bronchiolitis Obliterans Organizing Pneumonia

Epler1 recently presented an excellent
review of bronchiolitis obliterans organizing pneumonia (BOOP), and in this
classification, sarcoidosis is not described. Therefore, we thought it would
be interesting to report a case of sarcoidosis-associated BOOP whose radiographic
aspect is atypical, showing alveolar opacities with consolidations. This form
of alveolar sarcoidosis appears in less than 10% of cases,2-6
and its association with BOOP has not been previously described.
Report of a Case
A 45-year-old man was admitted to our institution 15 days after the
onset of illness with moderate-grade fever, nonproductive cough, and pleuritic
left-sided chest pain. He had had a tonsillectomy at age 12 years and smoked
(20 cigarettes per day) for the past 20 years, without other pathological
antecedents. Physical examination revealed an ill-appearing man, with an axillar
temperature of 37.8°C and normal blood pressure, pulse rate, and respiration
