Potential Impact of Sleep Disorder Treatment in Fibromyalgia Patients—Reply

We thank Krakow for his interest and comments concerning our recent article describing the efficacy of CBT for insomnia in FM patients.1 As his letter notes and our article acknowledged, our sleep laboratory screening of FM patients enrolled in our study did not include a full respiratory montage capable of detecting the subtle forms of sleep-disordered breathing mentioned in his letter. Thus, we agree with Krakow that future insomnia treatment studies with FM patients would benefit by the inclusion of thorough sleep laboratory screening for all forms of sleep-related respiratory disturbances.

However, we believe Krakow's letter conveys some important misconceptions that warrant comment. Of particular concern is his claim that CBT "largely affects psychological dimensions of insomnia, not physiological ones."
