In reply

We greatly appreciate the letter by Michalek and Cummings for bringing to our attention their publication on the relationship between smoking and age at diagnosis of various cancers, including CRC.1 It is reassuring that these cancer database reviews, conducted approximately 2 decades apart, both observed that smokers had an earlier age of CRC presentation. However, while the prostate cancer results were discordant, the explanation by Michalek and Cummings seems plausible. To date, the only other cancer we have investigated is pancreatic cancer. We observed that current smokers had a markedly earlier age at diagnosis compared with those who had never smoked.

We are also grateful to Anderson and colleagues for sharing results from their asymptomatic colonoscopy screening data. Their results, using a different methodology, provide important corroboration of our findings. Taken together, these lines of evidence strongly support the association between smoking and a younger age at diagnosis . . . [Full Text of this Article]
