Pitfalls in Diagnosing Chronic Kidney Disease From eGFR

Plantinga et al1 reported that only 9% of patients with chronic kidney disease (CKD) stage 3 (estimated glomerular filtration rate [eGFR], 30-59 mL/min/1.73 m2) were aware of having CKD, with awareness defined as whether the subject acknowledged having ever been told he or she had "weak or failing kidneys."

What is not mentioned is that by this definition of CKD stage 3, with the application of the Modification of Diet in Renal Disease (MDRD) equation, all white women older than 38 years with a serum creatinine as low as 1.1 mg/dL (to convert to micromoles per liter, multiply by 88.4), all white men older than 41 years with serum creatinine as low as 1.4 mg/dL, all African American women older than 38 years with serum creatinine as low as 1.3 mg/dL, and all African American men older than 49 years with serum creatinine as low . . .
