Risk Factors for Community-Acquired Infections Caused by Extended-Spectrum β-Lactamase–Producing Escherichia coli

In an important article, Rodriguez-Baño et al1 assessed risk factors for community-acquired infections caused by extended-spectrum β-lactamase (ESBL)-producing Escherichia coli urinary tract infections (UTIs), since the knowledge of factors associated with this emerging clinically relevant pathogen is scarce. Nonetheless, we are concerned about 2 issues in this study.
First, health care–associated infections are a relatively novel clinical entity distinct from community-acquired infections with characteristics that resemble those of nosocomial infections.2 Thus, it was surprising that patients with such infections were not excluded from the study. Even though this variable was controlled in a multivariate model, the statement that health care–associated infections and recent hospital admission are risk factors for community-acquired ESBL-producing E coli UTI is not compatible with the mean of community-acquired infection.
