Errors in Contributions. In the article titled "Frequency of Failure to Inform Patients of Clinically Significant Outpatient Test Results" by Casalino et al, published in the June 22 issue of the Archives (2009;169[12]:1123-1129), David O. Meltzer, MD, PhD, should have been included in the "Author Contributions" section on page 1128 for study concept and design; analysis and interpretation of data; and critical revision of the manuscript for important intellectual content. Also, Kari Fitzgerald Jerge, MD, should have been included in "Additional Contributions" for conducting medical record reviews and interviews.
