On the basis of a statistical analysis of our data, Drs Ong and Barnett have questioned the value of the test for antibody-coated bacteria to accurately localize infections of the urinary tract. We feel that their conclusions are not warranted, since our study was not designed to determine the ability of this test to differentiate upper from lower tract disease. We emphasized clearly that the conditions of our patients were separated into clinical syndromes that were not confirmed by direct localization procedures, since such procedures were generally neither indicated clinically nor performed. Even if we assume that our ability to clinically differentiate symptomatic upper from lower tract disease is perfect, and it is not, the assumption by Ong and Barnett that all of our patients with asymptomatic bacteriuria had lower tract disease is not justified.
