Forester's observation is very interesting. The rationale of magnesium in patients suffering electro-mechanical dissociation is not quite clear; nonetheless, magnesium might cause coronary vasodilation1 and increase myocardial perfusion on one hand, and, on the other, it improves myocardial performance as magnesium reduces peripheral vascular resistance.2
This observation merits further investigation. As for dosing, it seems that we would suggest giving a 2-g bolus form and, thereafter, 6-g bolus every 6 hours.3
